Identifying a clear and accurate transition plan is challenging and complex. As opposed to traditional transition assessments, ours carefully measure a client's ability, achievement, learning processes, values, and skills in the three main areas of adult life (learning, living, and working), as opposed to just gathering information about strengths and preferences. We believe that all of these factors are important in developing a complete road map that ushers teenagers into an adult life with deliberate intent. Our transition assessments are conducted by licensed psychologists.
Transition assessments are funded privately or through independent education evaluations (IEEs).
The transition between high school and adult life is a big one. We can provide consultation which helps to create, direct, and maintain a meaningful routine/structure that can either be sustained long-term or in the interim before a more permanent placement is started. Given the lack of placement options for adults with special needs, consultation can be one solution to help an adult child live with their family with more success.
We are also available to consult with attorneys, districts, treatment teams, and other professionals regarding transition needs and services. If you can't make it to our Pasadena office, we can also schedule remote consultation if you reside in the state of California.
We are available to evaluate Individualized Transition Plans (ITPs) and to help generate meaningful and results-oriented goals. Our professionals are also available to attend IEP meetings to work with IEP teams.
